UK Announces New Pathway to Promote Early Support for Patients with Chronic Conditions

 

 

The United Kingdom recently announced Diagnosis Connect, a new program designed to provide immediate support for patients diagnosed with long-term health conditions. By connecting individuals with relevant resources and organizations at the point of diagnosis, this service aims to empower patients with knowledge, facilitate early treatment and help guide patients to better care at an earlier stage. For those with an inflammatory condition such as rheumatoid arthritis, it might even enable them to achieve remission. 

Addressing an Urgent Need 

Currently, 1 in 4 people in the UK are living with two or more long-term health conditions. These individuals often face greater challenges, including a higher risk of complications and increased strain on healthcare services. Timely intervention is critical.  

Diagnosis Connect seeks to address this need by ensuring patients receive guidance and support from the moment they are diagnosed. By managing conditions early on, Diagnosis Connect hopes to reduce disease flare-ups, improve quality of life and avoid unnecessary hospital visits.   

Bridging the Gap for Patients with Chronic Conditions 

At its core, Diagnosis Connect recognizes the value of human connection in improving long-term health outcomes. The program focuses on bridging the gap between clinical care and community support, emphasizing early intervention and holistic care.  

Support groups will provide personalized advice, information and guidance as individuals navigate their diagnosis and its implications. Similar to how patients with cancer or dementia are referred to specialists and support services, Diagnosis Connect ensures those with chronic conditions receive the same level of care and assistance.   

Launching in 2026, Diagnosis Connect will initially support 250,000 people in its first two years. Priority disease states include rheumatic and musculoskeletal diseases, diabetes and respiratory conditions, with plans to expand to additional conditions over time. 

Supporting Patients with Chronic Inflammatory Conditions 

The Global Remission Coalition is committed to helping individuals with chronic inflammatory conditions achieve remission through early diagnosis, access to effective treatment and coordinated care. Diagnosis Connect aligns with this mission – introducing an integrated care model that emphasizes person-centered, holistic support.  

Deborah Alsina, Chief Executive of Versus Arthritis, emphasized how Diagnosis Connect will help the 450,000 people diagnosed with arthritis every year through early support and improved navigation of the health system.
