World AS Day: Not Only Back Pain

May 2 is World AS Day, also known as World AxSpA Day, an annual moment to stand with the more than 50 million people worldwide living with axial spondyloarthritis. This year’s theme, Not Only Back Pain, captures something those living with axSpA know all too well: What is often dismissed as ordinary back trouble is a complex, whole-body disease that deserves to be seen, understood and treated as such. 

The Burden of Axial Spondyloarthritis 

Axial spondyloarthritis is a chronic inflammatory rheumatic disease primarily affecting the spine, pelvis and sacroiliac joints. Alongside persistent pain and stiffness, people living with axSpA often experience debilitating fatigue and related conditions affecting the eyes, gut and skin. Many of these symptoms are invisible, making the disease easy to overlook and difficult to communicate to others.
 

The physical toll is exacerbated by economic strain. People with axSpA are three times more likely to prematurely leave the workforce, and the cumulative cost of lost productivity and absenteeism adds up. Uncontrolled disease activity affects everything from sleep to long-term independence, making clear that this is not only back pain, but a disease that shapes every dimension of a person’s life. 

The Importance of Early Diagnosis 

Because early symptoms like back pain, morning stiffness and aching in the hips can resemble more common conditions, axSpA is frequently missed. On average, diagnosis can take more than seven years from symptom onset. During that time, inflammation continues unchecked, structural damage accumulates and the opportunity for early intervention and treatment is missed. 

World AS Day is an opportunity to change that. When the hallmark signs of axSpA are recognized as red flags worth investigating, patients can access rheumatologic care before the disease causes lasting harm. 

The Value of Pursuing Remission 

An accurate diagnosis is the starting point, not the finish line. Around 75% of patients report their axSpA is not properly managed, which means the goal must extend beyond identification to actively treating the disease in pursuit of remission. When remission, or no disease activity, is reached, the difference is real: less pain, less fatigue, and a meaningful return to work, relationships and everyday life — with downstream benefits for productivity and the health care system too.  

World AS Day is a call to close the gap between the standard of care every axSpA patient deserves and the reality many still face. With greater awareness, earlier diagnosis and a shared commitment to remission, more people can move beyond simply managing axSpA. Instead, they can start living well with it.
